I could probably write an MSI installer and MSI Patch creation/installer, but that would be around 2 months of full-time work (I used to do that professionally). Microsoft really fowled things up, only thinking about enterprises, not small groups developing large, dynamic software.
It is a little easier for people developing VB.NET, because that is what the MS development tools are mainly designed for.
