It is a shame that now premiumax seems to try to block us as it just makes no sense.
...but anyways.
I just tested the plugin again in free mode and it seems to work.

From your log it looks more like serverside premiumax issues.
Can you ALWAYS download fine via browser via premiumax (same downloadlinks)?

At the moment I'd recommend you guys to use other (multi)hosters instead - sorry but it seems like this is what premiumax wants.
